Images upload... Sometimes

Greetings,
I've repeated an identical test 8 times on the same blog. I create a post, add the same image, put it in the same category and post it from the app.
Sometimes this image attaches itself, sometimes not. I JUST don't GET it. I've been on this for hours, publishing/unpublishing plugins (etc). BUT, this test I performed repeatedly, making NO CHANGES to the wp system.
HELP!?!?!?

You can follow the steps below and debug your site:
- Make sure you are running the latest version of WordPress (core)
- Disable all plugins
- Enable the logging in the xmlrpc.php file
- Try to upload a file by using the Android app
- Take a look at the webserver access log, and error log files.
- Take a look at the xmlrpc.log
