From what I can tell, I'd need an actual blog on WordPress.com to use the follow feature? I have a WP.com account and a self-hosted WP.org set up on the app, but I don't want to add a WP.com blog as well.

Yes, you'll need to have a WordPress.com blog in the app to use the Read feature in this version.
